Operator error caused the death of a Menasha teen on a carnival ride, according to a report by state inspectors. Elizabeth Mohl died after falling from the Air Glory ride at a Christian music festival in Oshkosh July 14th. Wisconsin Commerce Secretary Mary Burke, whose department inspected the ride immediately after the fatal incident, said the inspection did result in 25 code violations . Those findings concern Kris Potts, who served as a volunteer at LIFEST. Potts believes the ride should not have been allowed to operate. The owners of the Air Glory ride have been prohibited from Wisconsin for the remainder of this year, and will have to pass Commerce inspections on any rides it wishes to operate here for the next five years. Commerce has also established emergency rules for rides with carabineers, harnesses, and wires, and has launched a review of its amusement ride safety program.
