The Wisconsin Department of Health Services has found several violations at a Chippewa County health care facility. In a DHS report, which was obtained by WEAU 13 News, investigators found several violations at Lake Hallie Memory Care, a live-in center for Alzhemiers and dementia patients, when it was inspected last fall. The 30 violations include the hiring of a convicted sex offender.

The report states the facility did not properly background check employees during the hiring process. The male employee in question worked in and around the building for 7 months, and had day and night access to the building. The man was hired in February 2013, and fired the following September after the background check revealed his criminal history. The report also states Lake Hallie Memory Care violated several other health codes, including not ensuring their employees were properly trained to be working in a community based residential facility.

DHS says the company has completed a required plan of correction, which describes how the facility will fix the violations, and has paid a $20,000 fine. The company is not allowed to admit any new residents for the time being.
